Accommodation Information for SDS Students

This page has information about submitting your initial request for services through Accommodate. The information provided below is instructions for how to fill out your request.

First, go to the menu link “Registering with SDS,” and you will find an online intake registration form. Here, you must provide specific information about your diagnosis, access needs, and accommodations. Lastly, you need to provide supporting documentation of your disability/condition. After you have attached these forms and filled out the intake form, you can submit it to the Student Disability Services department. The review process can take as long as two weeks, so SDS can schedule an intake appointment after submitting your documentation. For more information, contact the SDS office.
