Accommodation Info: Getting registered with SDS

This page has information about submitting your initial request for services through Accommodate.  The information provided below is instructions for how to fill out your request.

First, you can go to the menu link “Registering with SDS” and here you will find an online intake registration form.  Here you need to provide specific information about your diagnosis, your access needs, and your accommodations.  Lastly you need to provide supporting documentation of your disability/condition. And, after you have attached these forms and filled out the intake form, then you can submit it to the Student Disability Services department.  The review process can take as long as 2 weeks, so after you have finished submitting your documentation, SDS can schedule an intake appointment.  For more information contact the SDS office.  Their phone number is 410-455-2459, or you can email them at
